Housing Case Manager

Department: Family Services Division
Location: Washington , DC

Under the direct supervision of the Program Manager, the Case Manager, will be responsible for the 50 participant assessment, provide solution -focused strength-based family centered case management services, triage incidents to determine the immediate action in terms of relocation, coordinate for pre and post Healing Circles, enroll the individual and families to various support services needed to meet the goals and development plan, and maintain all case notes and program data in ClientTrack software.

PRINCIPLE ACCOUNTABILITIES:

  • Home visits –Housed clients: a minimum of two contacts per month. Two (2) face-to-face contact per month, one must be in the clients’ home. Unhoused clients:1st 90 days in the program, a minimum of four contacts per month. Three (3) face-to-face contacts per month in the community. One (1) telephone, email, text, or in person. In program over 90 days; 2 home visits and 1 community or office visit.
  • Case ManagementIdentify and articulate critical issues confronting the client. Assist in identifying barriers, needs and strengths; developing goals; identifying resources and support; and connecting individuals with the needed resources, housing and/or economic security supports and supportive services to achieve identified goals.
  • Locating/engaging the client to explain the services provided by RRH and conducting a program intake;
  • Assessing the client’s housing barriers, needs, income, and preferences;
  • Developing a service plan (in coordination with the participant) for locating safe, affordable housing that meets participants’ needs in accordance with client intake practices and within funding requirements with a program exit strategy;
  • Connecting the client to relevant referrals and/or community resources;
  • Assisting with outreach to and negotiation with landlords/property owners;
  • Assisting with submitting rental and job applications

Education

Bachelor’s degree in social work, psychology, sociology, counseling, or related service/science or health care related disciplines and one (1 year) of experience providing case management services.

Experience

A minimum of two (2) years’ experience with homeless programs or provisions of social services. Knowledge of community resources. Excellent oral, written and interpersonal skills.

Additional Requirements:

Must be able to pass a drug and TB testing, Metropolitan Police Department and Federal Bureau of Investigation background check and Child Protective Services clearance. Covid vaccination required.
