Case Manager (100% IN OFFICE)

The Case Manager, under the direct supervision of the Program Manager and general supervision of the Family Services Director, is responsible for providing resources and referrals to families, conducting home visits, and advocating for families within the context of their team assignment (e.g. Intake and Assessment Team & Housing Team). The incumbent is responsible for maintaining documentation for their assigned cases and completing agency assessments, case plans, and other forms as necessary. S/he is also responsible for providing outreach services that employ prevention and intervention techniques and conducting collateral visits. Lastly, the incumbent will provide rotating emergency coverage.

PRINCIPLE ACCOUNTABILITIES:

Home VisitsConduct and schedule home visits with clients to monitor level of family functioning and stability to ensure housing, clothing and food needs are adequate. Build and maintain trust and rapport with families. Build on the strength of family to assist them in meeting their needs and respond to crisis and stress appropriately. Report incidents of child abuse and neglect and assist the family in modifying behaviors and/or conditions or refer to appropriate resources. 

Case Management Identify and articulate critical issues confronting the family. Review intake summary and in collaboration with the case manager, determine course of action(s). Regularly review data system to ensure the proper maintenance of documentation by staff of interactions among family members during home visits. Assist assigned social workers with obtaining the proper resources needed for clients. 

Resource and Referral – Investigate resources in the community to meet needs of clients. Ensure the maintenance of a current and up-to-date listing of all community-based programs, schools, civic associations, churches and businesses to assist clients in obtaining food, clothing, housing, employment, furniture etc. Provide housing relocation assistance for working with The Community Partnership and landlords. Ensure 24-hour accommodations for homeless client that provides rehabilitative and supportive services designed for stabilization. Maintain working relationships with outside agencies to promote the work of CSC, keep abreast of issues, and to advocate for client services.

Emergency Coverage– Provide emergency coverage to ensure support is available when clients are in need. Receive call and determine level of priority and respond as appropriate. Responds to critical situations in a timely manner.

Education

Bachelor of Arts Degree in Social Work, Education or Psychology or 2-3 years work experience in the field of social work and case management.

Experience

Two-year experience in community based social services or work in the areas of child development, family relations, and behavioral health or a B.S. in Social Work, Education, Psychology or a combination of education and experience.   Knowledge of community resources. Excellent oral, written and interpersonal skills.

Additional Requirements:

Must be able to pass a drug and TB testing, Metropolitan Police Department and Federal Bureau of Investigation background check and Child Protective Services clearance.
